3. QUALIFICATION OF DOCTORS

Our doctors are licensed Doctors of Chiropractic (DC) in the State of Florida:

• Dr. Douglas Kellerman, DC — Licensed Chiropractor, 26+ years experience
• Dr. Michael F. Petrie, DC — Licensed Chiropractor, 45+ years experience

They are NOT medical doctors (MDs) and cannot prescribe medications or perform surgery. Chiropractic care is a complementary health approach.

4. DESCRIPTION OF SERVICES

Spine & Joint Center offers chiropractic services including but not limited to:

• Spinal adjustments (various techniques: Diversified, Gonstead, Activator)
• Corrective chiropractic care
• Physical therapy and rehabilitation
• Spinal decompression
• Acupuncture (as licensed)
• Preventive wellness care

Services are provided by appointment only. Availability depends on doctor schedule and case severity.

5. PATIENT RESPONSIBILITIES

By scheduling an appointment, you agree to:

• Provide accurate health history and current symptoms
• Disclose all medications, supplements, and existing conditions
• Inform doctors of any allergies or sensitivities
• Follow post-care instructions provided by the doctor
• Attend appointments as scheduled or provide 24-hour cancellation notice

6. LIMITATION OF LIABILITY

TO THE MAXIMUM EXTENT PERMITTED BY LAW:

8. FINANCIAL TERMS

Payment Terms:

• Payment is due at time of service unless prior arrangements are made
• Accepted payment methods: cash, credit card, debit card, insurance
• Insurance: We verify coverage before your visit when possible; you are responsible for copays, deductibles, and non-covered services
• Late payments may result in collection action
• Refunds are not provided for completed services except at doctor discretion in cases of billing error

Cancellation Policy:

• Cancellations require 24 hours notice
• Late cancellations (less than 24 hours) may result in a $50 cancellation fee
• No-shows may be charged the full appointment fee

9. LIMITATION ON MEDICAL ADVICE

Information on our website or provided via email/phone is NOT:
