Our treating doctors are Dr. Douglas Kellerman, DC (Florida licence CH8001) and Dr. Michael F. Petrie, DC (Florida licence CH3310).
2. Information you give us
The appointment form on our Schedule page and the form on our Contact page collect the following:
- Name — so we know who is asking
- Phone number — so we can call you back to arrange your appointment
- Email address — so we can confirm and follow up
- Message — anything you choose to tell us
When you submit one of these forms, its contents are emailed to info@spineandjointcenterfortlauderdale.com and to drkellerman@gmail.com so that our team can respond. Our email is provided by Google Workspace. If you reply to that email, your reply is sent to our inbox.
Providing this information is voluntary, but we need at least a name and a way to contact you in order to arrange an appointment.

7. How long we keep it
Appointment enquiries that do not become patient records are kept for as long as needed to deal with your enquiry and for a reasonable period afterwards for our own records. Where an enquiry becomes part of your clinical record, it is retained under our HIPAA Notice and applicable Florida record-retention requirements, which are longer. Analytics data is retained according to the retention period configured in Google Analytics.
8. Your choices and rights
You can ask us to:
- tell you what personal information we hold about you through this website;
- correct anything that is wrong;
- delete information that we are not required to keep.

9. Children
This website is intended for adults arranging their own or a family member’s care. We do not knowingly collect personal information from children under 13 online. If you believe a child has submitted information through this site, contact us and we will remove it.
